Job Title: Qualified Mental Health Professional (QMHP-A)

Reports to: Site Director

Duties and Responsibilities

The QMHP-A is responsible for the provision of direct care to adults and clients and provides skill-building training to assist them in the community and stabilize their level of functioning. QMHP-A is responsible for all clients related to direct service documentation and reports, as well as the development of the Individual Service Plans (ISP). The QMHP-A (Mental Health Support Worker) facilitates skill-building to assist clients with increasing their independent living skills, provides support to stabilize mental health issues and links to various community services, and advocates for the need of clients. The QMHP-A allots 95% of the time to direct care and provides documentation to demonstrate delivered services to clients. QMHP-A participates in monthly supervision/training to improve and enhance skill levels. The position reports to the Site Director and is under the direct oversight of the Lead QMHP. Services are delivered in the office, community, and other various locations.

Knowledge, Skills, and Abilities

  • Knowledge of some characters and concepts of mental illness and related physical conditions and treatment approaches.

  • Ability to engage in the care, training, and rehabilitation of physically and mentally challenge individuals and provide assistance and advocacy to their needs.

  • Ability to observe the rights and personal dignity of others.

  • Ability to observe, record, and report clients’ behavior, attitude, and physical condition.

  • Prepare a written individual service plan, incorporating input from clients, involved service providers, and clearly identifying problem areas and needs, strategies, and service objectives.

  • Link clients with external programs or services, such as health services, recreational activities, financial resources, employment resources, AA/NA groups, transportation resources, and others.

  • Collaborate and coordinate with other service providers and professionals, such as doctors, social workers, case managers, and probation/parole officers.

  • Complete all progress notes and quarterly reports in a timely fashion.

  • Conduct discharge planning by the program description.

  • Attend all meetings and training’s as required.

  • Ability to maintain effective working relationships with clients and other employees. Requirements Skills:

  • Knowledge of DSM V Diagnosis

  • Case management/Documentation

  • Oral and written communication

  • Individual Services Plan development

  • Weekly and Quarterly Documentation

  • Crisis intervention

  • Ability to maintain confidentiality

Education and Experience

  • a bachelor’s degree in human services or related field (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling, or other degree deemed equivalent to those described) from an accredited college and with at least two years of clinical experience providing direct services to individuals with a diagnosis of mental illness or

  • at least a bachelor’s degree from an accredited college in an unrelated field that includes at least 15-semester credits (or equivalent) in a human services field and who has at least three years of clinical experience (but needs to complete training hours as per new regulations)

Required Training and Certification

This position requires the individual to be certified and attended the training.
  • Certified CPR and First Aid

  • Behavioral Management Training certified

  • Certified QMHP in VA by DHP

  • Note: Background check and TB screening will be done within 30 days of employment.

Job Title: Qualified Paraprofessional in Mental Health (QPPMH)

Reports to: QMHP /Site Director

Duties and Responsibilities

The Qualified Paraprofessional is responsible for the provision of direct care to adults and provides support and training to assist them in the community and stabilize their level of functioning. Paraprofessionals are responsible for all clients related to direct service documentation and reports, as well as the development of the Individual Service Plans (ISP). The Paraprofessional facilitates skill-building to assist clients with increasing their independent living skills, provides support to stabilize mental health issues and links to various community services, and advocates for the need of clients. The Paraprofessionals allots 95% of the time to direct care and provides documentation to demonstrate delivered services to clients. Paraprofessional will be supervised by a QMHP and reviews ISP participates in weekly supervision/training to improve and enhance skill level. The position reports to Site Director and is under the direct oversight of the (Lead) QMHP-A. Services are delivered in the office, community, and other various locations (Services provided by qualified paraprofessionals require supervision of a Qualified Mental Health Professional (QMHP). Supervision is demonstrated by the QMHP by a review of progress notes, the member’s progress towards achieving ISP goals and objectives, and recommendations for change based on the member’s status. Supervision must occur and be documented monthly in the clinical record)

Knowledge, Skills, and Abilities

  • Knowledge of some characters and concepts of mental illness and related physical conditions and treatments approaches

  • Knowledge of simple nursing care, first-aid, behavior management, personal and environmental hygiene.

  • Ability to implement and follow the policies and procedures of DBHDS and RHCSS

  • and meet compliance.

  • Ability to engage in the care, training, and rehabilitation of physically and mentally ill clients and provide assistance and advocacy to their needs.

  • Ability to perform established training, care, and programmatic activities.

  • Ability to participate with professional staff in the design and implementation of training and programmatic activities.

  • Ability to observe the rights and personal dignity of others.

  • Ability to observe, record, and report clients’ behavior, attitude and physical condition.

  • Ability to perform simple math and communicate effectively, both orally and written.

  • Ability to maintain effective working relationships with clients and other employees.

Education and Experience

Must meet DBHDS qualifications.
“Qualified Paraprofessional in Mental Health (QPPMH)” means a person who must, at a minimum, meet one of the following criteria:
  • Registered with the United States Psychiatric Association (USPRA) as an Associate Psychiatric Rehabilitation Provider (APRP);

  • Has an associate’s degree in a related field (social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, human services counseling) and at least one year of experience providing direct services to individuals with a diagnosis of mental illness; or

  • Has a minimum of *90 hours’ classroom training and 12 weeks of experience under the direct personal supervision of a QMHP-Adult providing services to individuals with mental illness and at least one year of experience (including the 12 weeks of supervised experience).*
